Aztech Lab was a Canadian company.
They shipped some Audio+Modem combo cards.
I own one detected as AZT3002 PNP by Windows 95 (PNPID: AZT3002) while it has
been marketed as Aztech AT3300-E (AT for Audio Telephony - E for Europe).
The ones with a PNPID of AZT3001 are AT3300-U (U for USA).
It seems it was used in some HP, Packard Bell and Compaq machines.

The sound card is installed in the scenic pro and I use
the Universal ISA PnP Sound Card Driver for DOS
(UNISOUND81b.zip) with the following
configuration in autoexec.bat
:
REM A2x0 = Memory address port (220, 240, 260, ...)
REM Ix = IRQ used by the sound card (5, 7, 11, ...)
REM Dx = DMA channel (0 or 1)
REM Tx = Sound card type (1=SoundBlaster, 2=SB1.5, 3=SB2, 4=SBPro, 5=SBPro+FM, 6=SB16)
REM Jxxx = Joystick/game port (xxx=hex port). J0=Disable.
REM Pxxx = MPU (MIDI) port address (330 is default). P0=Disable.

Note: I was unable to achieve FM sound under MS-DOS either with UNISOUND.COM 8.1b
